Before a Producer can sell, solicit or negotiate dental insurance on behalf of Security Life Insurance Company of America, the producer must be contracted and appointed to Security Life in the state(s) in which they intend represent the Company.
Complete the following documents in their entirety and mailed them along with a copy of a current license and appointment fee to:
