PrimeStar Dental Plan Customer Service
Welcome to the PrimeStar Dental Plan Customer Service on-line assistance page, dedicated to providing easy and efficient service to our current dental insurance customers. The following are answers to some of the common questions/concerns with your dental insurance plan.

Insurance Certificate and Dental Identification Card (Welcome Kit)
New applicants are mailed a Welcome Kit which contains your certificate of insurance and a dental identification card with in 3 to 4 weeks from the date of submission. If you have not received a Welcome Kit within this time, call our administrative offices: 877-862-8949

Insurance Certificate ‚ describes benefits, guidelines and exclusions of your dental coverage. The certificate is the basis for the payment of all claims. You should review this certificate to become familiar with your plan and retain for your records.

Dental Identification Card ‚ is proof of dental insurance coverage. You will need to show this card to your dental service provider upon receiving dental services. If applicable, the provider will use the information on your card to submit a claim. This card also contains the customer service number for any questions.


Enrollment/Change Forms
Use this form to notify us of a status change in your coverage. A status change can be a change of address, marriage, divorce, death or newly acquired dependent. Click here to download the Enrollment/Change form to make the following changes to your certificate:
  • Change in Status (termination, addition, etc.)
  • Address Change
  • Name Change
  • Add/Remove a Dependent
Claim Form
Generally, PrimeStar dental claims are submitted by the dental service provider on your behalf. Most providers have a supply of claim forms. If necessary you may download a claim form here for completion and submission. Click here to download a claim form.
