Q: Can an insured add the vision coverage to their PrimeStar plan after they initially declined the vision rider?
A: Yes, an insured can add the vision rider at any time. They must complete a change form requesting the change. New premiums would be charged to reflect this new coverage. The benefit waiting periods for vision coverage would begin on the date in which vision coverage became effective.
Q: Do the PrimeStar plans coordinate with other plans?
A: Yes, we will coordinate our plans with other group insurance plans.
This provision does not apply in Maryland and South Dakota.
Q: Is a non-US citizen eligible to enroll in the dental program?
A: Yes, all we require is a US address.
Q: Are there any age restrictions on the PrimeStar Plan?
A: The PrimeStar plan provides different rates for applicants age 65 and older. If an applicant is under age 65 prior to enrollment we will not increase rates solely upon reaching age 65. Subsequent rates will continue to be based off of the under age 65 rate table.
Q: What is the minimum age that an applicant must be to apply for the PrimeStar plan?
A: 18 years of age.
Q: If someone enrolls under the dental as applicant only, can he purchase vision as a family plan?
A: No, the same coverage level will apply to both the dental and vision coverages.
Q: How is the effective date determined?
A: The effective date is the first of the month following the day in which the application is received in the Service Center office. We will allow up to five days into the requested effective month for applications to be submitted provided the application date is prior to the effective date. Example: If we receive an application on June 4th, we will allow an effective date of June 1st, providing the application date is prior to June 1st. The group/individual should NOT cancel any other coverage until they receive written acceptance of coverage from our office.
Q: On which day of the month does the Bank Account Debit (ACH) and Credit Card payments draft?
A: All bank drafts (checking, savings and credit card) take place on the first of the month.
Q: Is eligibility up-dates process done timely for claim purposes?
A: The claims adjudication system is updated nightly with all eligibility information. If necessary, both the billing system and claims system can be checked if an insured cannot be located.
Q: When can a new family member be added to the plan?
A: If a new dependent is acquired (birth, adoption, marriage) that dependent may be added to the plan immediately as long as the change occurs within 31 days of the qualifying event. A change form must be completed indicating the change, waiting periods will apply. Click here for a PDF of the change form.
Q: Will you pay claims on services rendered in Mexico?
A: Services must be performed by a licensed dentist acting within the scope of the license. Services must be rendered by a Mexico, licensed dentist.
Q: Are These Plans Fully Insured?
A: Yes. The Dental Plan is fully insured by Security Life Insurance Company of America. Security Life is headquartered in Minnetonka, MN, at 10901 Red Circle Drive. Over the last forty years, Security Life Insurance has earned an outstanding reputation as a leader in the field of ancillary health and wellness insurance benefits, such as dental and vision plans.
Q: Is a Company check acceptable on PrimeStar?
A: Yes.
Q: Can I get credit if I was covered under a prior dental plan?
A: Credit (CPT) is not an option on the personal plan. The only exception is for insureds who are currently insured by a Security Life group plan and they wish to elect personal coverage themselves. However, the insured must submit a written request to Security Life for CPT benefits. Availability and rates will be determined at that time.
Q: Is the deductible cumulative?
A: Deductibles vary by service. Deductibles for Preventive services are per lifetime of insured. Deductible for Basic and Major are per calendar year per insured. However, there are maximum deductibles illustrated on the inside cover of both brochures.
Q: When can I make changes to my plan
election or calendar year maximum?
A: You can change plans (ie. move from the
Select Plan to the Premier Plan) or increase/decrease your Calendar Year
Maximum amount on your annual anniversary date.
PrimeStar Personal Dental Plans will reimburse you for covered dental expenses based upon the Reasonable and Customary (R&C) fees for those covered expenses.
